Provides assessment, brief psychotherapy and psycho-education for Geisinger at Home (GaH) patients under the clinical supervision of the GaH Psychologist and GaH Advanced Practitioners. Administrative supervision will be coordinated by Director, Clinical Management, GaH. Functions as a member of the GaH treatment team providing input regarding diagnosis and plan of care. Provides information/education about plan of care to the patient, family and involved professionals. Serves as consultant to GaH team about behavioral health issues and services.
Functions as a member of the GaH Treatment Team - evaluates the ongoing bio-psychosocial needs of patients and relays information to other team members for incorporation into the plan of care. Regularly collaborates with other team members to promote fully integrated care.
Obtains and documents pertinent data as required for quality care, Geisinger standards and regulatory requirements. Maintains awareness of GaH key metrics and supplies data as requested.
Participates in and at times provides training for the expanded GaH care team.
Establishes and maintains effective relationships with other professionals and organizations- both internal and external.
Provides treatment, behavioral health counseling, and care within GaH in accordance within an integrated care model for patients served by Geisinger and its community partners. This includes any and all services consistent with a patient centered approach.
Skills and Abilities:
Demonstrated knowledge of behavioral health concepts and terminology, including working knowledge of DSM V.
Demonstrated knowledge of individual and group behavior and the relationship among social, economic, psychological and physiological factors.
Ability to identify and effectively utilize community resources.
Demonstrated organizational and communication skills.
Demonstrated competence in time management and performance as a team member.
Demonstrated personal maturity, flexibility and adaptability.
Demonstrated basic knowledge of psychopharmacologic agents.
Demonstrated knowledge of computer skills including Word and Excel.
Education and/or Experience:
Master's Degree in social work, psychology, nursing or other related discipline from an accredited university with a minimum of 3 years related experience required. Professional-related Pennsylvania licensure required as a LCSW. Previous treatment experience with individual, group, or family psychotherapy required.
Working Conditions / Physical Demands:
Work will be performed in patient homes as well as telephonic/office/clinical environment.
