Position Overview : The Clinical Integration Specialist will lead the clinical operations focus of practice integrations, leveraging experience and skill sets to manage and facilitate the successful integration of varying size office practices to CareMount Medical.
Essential Duties and Responsibilities: (including but not limited to the following)
Develop and maintain a CMM Best Practice approach to Clinical Operations. The approach will be designed and maintained with the collaboration and feedback of a multi-discipline workgroup.
Develop and maintain the Clinical Operations integration process and methodology.
Manages the day-to-day clinical operations aspects of the integration project
Facilitates, coordinates, and schedules site visits for varying CMM team members
Collaborates with other segments of the integration team including Information Technology, Human Resources, Purchasing and Finance to facilitate all aspects of the integration as they relate to clinical operations.
Acts as primary on-site liaison with integrating practice members and provides consultation and information on site for integrating physicians and care team members.
Collects, evaluates and outlines all clinical work flow processes of existing practice/location/specialty. The current state assessment will then be compared against the CMM Best Practice to identify 'gaps' in workflow process or knowledge.
Collaborate with the Integration Team to develop a plan of action to remediate workflow gaps - this will include workflow, change management, physician alignment, management alignment and a personnel review.
Collaborates with integrating practice management team to obtain and review any written clinical job descriptions, policies, and procedures.
Evaluates existing care team members in conjunction with the integrating practice management team to identify any performance or workforce issues. Do you want this person evaluating staff? Or do you want them focused on the workflow and the operational flow? Or maybe both - I wouldn't think they replace the managers responsibility (maybe a contributor to the Managers assessment).
Effectively applies standardized CareMount Medical integration processes and methodology.
Contributes to the Integration / Project Work Plan to ensure that all of the apporiate Clinical Operational components are accounted for and aligned with other work efforts. Is accountable for integration-related communications with the integrating practice management team and the CareMount Medical integration team relating to Clinical Operations.
Participates in interview(s) and offer(s) for integrating practice care team.
Facilitates and coordinates the provider onboarding process with site leadership and CareMount Medical integration team.
Collaborates with HR in coordination of staff on-boarding process, including employee orientation.
Collaborates with Learning Services team for on-boarding and orientation of all clinical roles including providers.
Collaborates with internal business owners on any process improvements designed to support business goals.
Collaborates with multi-disciplinary teams on planning and initiation of new service platforms to be added to new practices, i.e. Patient Prompt.
Collaborates with integration team for planning of go-live support.
Performs related work as required.
Adheres to all CareMount Medical Group's policies and procedures including OSHA and HIPAA guidelines.
Knowledge of CMM procedures for varying specialties
Ability to establish and maintain effective working and collaborative relationships with integrating practice managerial/supervisory team members, as well supervisors and managers from other CareMount departments.
Ability to foster a patient-centered environment.
Ability to educate and facilitate with regard to necessary changes.
Ability to handle stressful situations.
Typical Physical Demands:
Work requires sitting for long periods of time; also stooping, bending and stretching for files and supplies. Occasionally lifting files or paper weighing up to 30 pounds.
Requires the use of office equipment, such as computer terminals, telephones and copiers.
Travel to various practice locations and may require overnight travel during high activity periods, including Go Lives.
Typical Working Conditions:
Work is performed in a variety of settings and offices.
Involves frequent contact with staff, manager(s) and supervisor(s) both from integrating practices and within CareMount Medical.
Work may be stressful at times, interaction with others is constant and interruptive

Skilled and has Experience with delivery of Process Improvement Methodologies - Lean, Six Sigma
Superior management and organizational skills including the ability to lead, coach and facilitate change.
Excellent written and oral communication skills
Self-starter with the ability to work independently
PC proficiency with Microsoft Office Suite
Minimum of 8-10 years management experience preferably in healthcare environment, with demonstrated success in managing change and motivating a large team.
BA/BS/BSN degree in Nursing, Business Administrated or related field Master's Degree preferred; preferably in Business Administration, Healthcare Administration, or Communication
