Pharmacy Patient Insurance Coordinator, Outpatient Pharmacy - FT/Days (8hr)

Position Summary

The Intake Coordinator is responsible for the processing of new referrals, verification of insurance coverage, both new and ongoing, and coordination of home infusion services to patients/clients. Collaborates with the Clinical Pharmacists and Technicians in resolving patient care issues, and acting as a liaison between home health agencies and case management/utilization review departments. The Intake Coordinator plays a key role in quality assurance monitoring for infusion services.

Essential Functions and Responsibilities of the Job

• Coordinates infusion and nursing services for the pharmacy.
• Handles all incoming calls regarding new referral.
• Takes new referral phone calls.
• Calls all new referral sources to acknowledge fax have been received.
• Documents all communications from the case managers, nursing agencies, patient/caregiver, physicians and pharmacists into the computer system.
• Responsible for notifying the pharmacy as soon as possible with all referral updates.
• Secures nursing when required by referral source
• Verifies insurance coverage and creates patient demographics.
• Requests authorizations for requested services.
• Creates documentation for compliance purposes.
• Contributes to ongoing communication with the home health agencies, hospitals, and doctor’s offices for the pharmacy.
• Notifies the referral source of the nursing agency assigned to the case.
• Responsible for communicating with the patient/caregiver in introducing MemorialCare Infusion Care Pharmacy to the family
