Work Hours/Shift: Full Time, Days Monday-Friday You Will Be Responsible For: - Performs and facilitates activities and functions involved in the Health Assistant job responsibilities to maintain efficient operations
- Assist in reviews of daily reporting and tasks to provide support to Community Care Nurse Health Advisors and all other support staff
- Complete educational mail outs in a scheduled and timely manner
- Assists in implementation of new processes within the Community Care department, evaluate current processes to determine appropriate use, provide assistance to Community Care staff to improve performance and productivity
- Assists and updates management on work-related issues, such as risk management, cost containment concerns, interdepartmental and/or intradepartmental concerns and medical staff issues. Assists in the preparation of cases to be reviewed by Supervisor, Director, and/or Medical Director.
- Ensure file/records are maintained in accurate, current, organized, detailed, and confidential manner. Accurately completes all data entry and/or documentation relating to a precertification or authorization. Meets standards for documentation completion and accuracy as measured in regularly scheduled audits.
- Answers telephones, conduct triage as appropriate, and route calls to appropriate staff
- Provide and coordinate appropriate community referrals
- Assist members, as appropriate, with navigating and completing online form applications (e.g. Medicaid, transportation assistance)
- Participates in Community Care report preparation, analysis, and/or census management, service metrics, and payor reporting. Participates in committees, teams or other work projects/duties as assigned.
- Demonstrates regard and maintains confidentiality of all information for Case Management process in accordance with AHS policy and HIPAA regulatory requirements.
Qualifications
What You Will Need: KNOWLEDGE AND SKILLS REQUIRED: - Comprehensive understanding of the Community Care and Population Health purposes
- Ability to work in a fast-paced setting
- Excellent interpersonal skills
- Excellent written and oral communication skills and organizational skills
- Must demonstrate a passion for assisting Nurse Health Advisor(s) with assisting clients with health improvement and navigation of the health care system and programs
- Must demonstrate the ability to problem solve and work independently with solid judgement
KNOWLEDGE AND SKILLS PREFERRED: - Computer knowledge/skills to include spreadsheets, email, and presentation software Preferred.
- Familiarity with HIPAA and confidentiality practices Preferred.
- Knowledge of medical terminology Preferred.
- Knowledge of crisis intervention and cultural sensitivity Preferred.
EDUCATION AND EXPERIENCE REQUIRED: - High School diploma or equivalent required
- 2+ years of experience of general clinical setting
- AS/Wellness degree preferred
LICENSURE, CERTIFICATION OR REGISTRATION PREFERRED: - Prior experience in a human service field or as an administrative assistant/secretary
- Work history in an ambulatory/specialty clinic is a plus
Job Summary: The Health Assistant is responsible for providing Nurse Health Advisor support from the time of referral through the entire Care Management Program process in the Community Care program. This is accomplished by performing various functions, including but not limited to, data entry, filing, obtaining records, scheduling member appointments with care managers, mailing education packets, opening and closing cases, and assisting with outcomes reporting. Adheres to AdventHealth corporate compliance plan and to all rules and regulations of all applicable local, state and federal agencies and accrediting bodies. Attends applicable meetings to maintain up-to-date knowledge base as it relates to the Health Assistant activities.
