The Medical Instrument Technician (Vascular) is directly responsible for the day-to-day operation, administration, strategic development, and overall implementation of policies in the noninvasive vascular diagnostic lab, plus supervision of other technologist employee(s) in the vascular laboratory at the WRJVAMC. Demonstrated Knowledge, Skills and Abilities: A high degree of technical skills are required in the following areas: Cerebrovascular exams: carotid & vertebral extracranial duplex examination, transcranial Doppler examination of intracranial arteries, and to the extent technically possible, duplex evaluation of intra-thoracic carotid, innominate and subclavian arteries. Upper extremity exams: Duplex evaluation of axillary, brachial, radial, ulnar and palmar arch arteries. Duplex evaluation of all major upper extremity veins. Duplex and physiologic evaluation of arteries and veins for pre-hemodialysis access planning. Physiologic pressure measurements of brachial arteries, forearm arteries and digital arteries. Abdominal visceral duplex exams: Renal arteries and accessory renal arteries. Renal veins. Celiac, Superior Mesenteric and Inferior Mesenteric arteries. Superior mesenteric, splenic, portal and hepatic veins. Abdominal aorto-iliac duplex exams: Aortic duplex for occlusive disease and aneurysm diameter assessment. Common and External Iliac artery duplex for occlusive disease and aneurysm diameter measurement. To the extent technically possible, duplex evaluation of internal iliac arteries. Abdominal vena cava and iliac vein duplex: To the extent technically possible, duplex evaluation of patency of inferior vena cava and iliac veins. Lower extremity exams: Duplex evaluation of common femoral, Profunda femoris, superficial femoral, popliteal and tibial arteries for occlusive disease. Duplex evaluation of lower extremity arterial bypass grafts for patency and stenosis. Duplex evaluation of femoral, popliteal, and tibial veins for deep venous thrombosis. Duplex evaluation of femoral, popliteal and tibial veins for evaluation of venous valvular insufficiency. Duplex evaluation of greater and short saphenous veins for potential use as bypass conduits. Physiologic evaluation of femoral, popliteal, tibial and digital artery pressures as well as Doppler waveform capture and analysis. Instrumentation Skills: Maintains expert working knowledge of color-flow duplex scanner, arterial physiologic assessment systems (e.g. Parks Flow-Lab), treadmill for intermittent claudication studies. Data Presentation: Provides detailed preliminary data and working draft analysis as basis for physician interpretation. Mobility: Ability to perform inpatient studies in situations where the patient cannot be safely transported, e.g. examinations in ICU and in Operating Room - Computer Skills: Assists in implementation and maintenance of a digital Picture Archiving and Computer Storage (PACS) system for the vascular laboratory. Creates and maintains computerized template medical notes and structured laboratory reports for the VA electronic medical record. Quality Assessment: Establishes and maintain a quality monitoring system that surveys study quality and identifies critical incidents and sentinel events for action. Creation and maintenance of study protocols for all commonly performed exams. Ability to address variations from standards of care.
